There is probably a better way, but the way I have always done it is quite simple!  I just use Word and the Microsoft Clipart gallery images. (Formatting - comic sans with double spacing between words for easier readability, landscape orientation, and .5 inch margins all around) I have made a few just for our kids that incorporate their own photos.  Photos are also fairly straightforward to insert into the Word doc.  Once you have your text and images, print as a PDF.  Easy Peasy =)
